This is a short survey with questions about your organization. We will use your responses to build a PROFILE PAGE for your organization. Upon completion, you will be entered into a raffle for $500.00 (Q4,000.00) as one of the first 500 organizations to fill out a survey. Drawing will be held December 2008. Fill out your profile survey now to guarantee your spot in our raffle!
[NOTE: If you do not wish to respond to any of the questions, you do not have to. This is your profile and your representation of your organization. The only required questions are demarcated with a red star in the survey.]
After we process your responses to the survey, we will send you a temporary profile for your approval or for any changes that you would like to make. When our technology is fully functional, you will be able to edit your profile page directly from you account.
It’s easy and free to participate. The only cost is 15 minutes of your time.
